CCMH Medical Clinic - Main Campus

Crawford County Memorial Hospital offers a wide variety of quality health care services. Our skilled, experienced and knowledgeable staff work hard to provide compassionate care with a personal touch. When you utilize our services, we’ll show you and your family how we care for life.

From prenatal to geriatric care, to minor office procedures, CCMH Medical Clinic – Main Campus provides a full scope of family medicine services for patients of all ages. With six board-certified/board-eligible family medicine physicians and six advanced medical practitioners, the Main Campus staff is proud to serve the residents of Crawford County. CCMH Medical Clinic – Main Campus is a certified Medicare and Medicaid provider-based rural health clinic (RHC).

Request an Appointment

You can submit an appointment request to CCMH Medical Clinic – Main Campus using this online form. After you have submitted your request, one of our schedulers will contact you within 24 hours by phone to confirm an appointment or suggest a different time if necessary. Requests made Friday evenings or on weekends will be answered the following Monday.

Your requested appointment must be at least 48 hours from the current time. If you need to see a doctor sooner, please call us at 712.265.2700.

Attached to the hospital is the CCMH Medical Clinic, which is a provider-based rural health clinic (RHC). The CCMH Medical Clinic currently has four full-time family practice physicians.

Consent For Treatment

As many parents know, sometimes when they are out of town their children have an accident or develop an illness that requires medical treatment. In the event that happens and the caregiver watching the children needs to seek medical treatment, a parent should fill out a consent for treatment form and fax it back to the clinic at 712.263.1777 to authorize the necessary action.
