Crawford County Health Survey now available


Crawford County Memorial Hospital – along with Crawford County Home Health, Hospice & Public Health and other community partners – has developed a survey to learn more about health and the quality of life in Crawford County. The information from this survey will be used to identify our county’s most pressing community health problems.

We are asking for volunteers to complete this anonymous and confidential survey. We would like to have a minimum of 200 individuals fill this out. Participants should be 18 years of age or older.

We have developed both an online survey and a traditional paper/pencil survey. The online survey can be accessed here.

The paper survey is available at the following locations – CCMH main lobby, Family Medicine Associates lobby, Crawford County Home Health, Hospice & Public Health, Denison Medical & Surgical Associates, Broadway Dental, Thrifty White Pharmacy and West Iowa Community Mental Health Center. We plan to add additional locations throughout the county to this list.

We would like to have all survey responses back by November 5, 2010. Please help spread the word to friends and family members who live in Crawford County. We would like to have a representative sample with equal numbers of men and women, along with participation from adults of different ethnicities and age ranges.
