Medical Records |
As a new patient, you will be asked to provide information about your medical history. Please take your time and complete the information sheet thoroughly. Please notify us of any address, phone number or insurance changes. The information in your chart is confidential and the entire staff will protect that confidentiality.
If you wish to have your records released to an insurance company, lawyer or another physician, you will need to sign an authorization to release medical information. We will the information for you.

| Appointments |
Due to physician schedules, we ask that you call several days in advance to make an appointment with the physician you wish to visit.
If you need to be seen the same day and your regular provider is unavailable, you may see any one of the other providers in our office. Please bring the names of any medications you are taking which may help the physician with your care.
Patients are seen according to their appointment times and not according to the order in which they arrive. All attempts are made to minimize waiting but emergencies do occur and disrupt even the best schedule. Your patient is appreciated during these times, and if it is more convenient , another appointment will be given.

In Case of Emergency |
For an emergency during office hours, please call the office and state that you have an emergency. The nurse or physician will speak with you immediately and give you further instructions.
If an emergency occurs after hours, you may also call the operator at Crawford County Memorial Hospital - Denison at 712-263-5021. The operator will contact a physician on call who will return your call quickly. If you need to be seen, the physician will direct you to go to the Emergency Department at Crawford County Memorial Hospital-Denison.

Thursday - Friday 9:00a.m. - 5:00p.m.

Prescription Renewals |
Please call your pharmacy for prescription refills. In most cases the pharmacist will refill the prescription for you. However, you may need to see the physician before the prescription can be refilled. Please call the pharmacy before you are out of medicine to allow sufficient time to prepare your prescription.

Telephone Consultations |
If you are having a problem and would like to talk to the provider or nurse during office hours, please call us. If neither is available at the time you call, we will take your name, phone number and reason for calling. This information will allow more efficient attention to your problem. The nursing staff has been trained to answer many of your questions and will help you whenever possible.

Billing Procedure |
You are required to pay your coinsurance and/or co-payment at the time of service. Payments may be made in the form of cash, check or bankcard (Visa, MasterCard). Our billing office will file your insurance claim for you. Please notify us immediately of any changes in your address, phone number or insurance information. Please bring your insurance cards with you for every appointment.
