Dr. Scott D. Hoffman, D.O. is a native of Carroll, IA. Dr. Hoffman joined
the Family Practice and Surgical Specialists staff on August 1, 2004 and
is board certified in Family Practice. He received his Bachelor of Science
degree from Iowa State University in Biology and received his Doctor of
Osteopathic Medicine from Des Moines University. He completed his family
medicine residency at Mercy Medical Center North Iowa in Mason City. Dr.
Hoffman and his wife Kaycee have two sons, Jackson and Benjamin and one
daughter, Mallory.
