Crawford County Memorial Hospital is committed to making each
patients experience a positive one. Understanding insurance and billing
procedures is a part of that process.
Many insurance companies require pre-authorization before an individual
is eligible for full benefits. Each company is different, but many offer
an 800 number which can generally be found on the back of your insurance
card. Call this number for pre-authorization, prior to receiving care,
so you can receive the maximum benefits from your insurance policy.

You can help by:
1. Providing Copies of your insurance card.
2. If you haven't heard anything from your insurance carrier
in a
reasonable time, give us or them a call. (This may alert to a
billing problem.)
3. Other financial aid is available, call us and we can assist
with names
and phone numbers.
4. Keep us informed. We are willing to work with you.
5. Consider pre-payment of upcoming hospital services.
